Changes to CCB License or Records
Record Change - Address, ABN, Entity
     Use this form to make changes on your CCB license record for your address of record,  
     assumed business name*, or changes to your entity*.
 
Record Change – Endorsement
     Use this form to make changes to the license endorsement. Contractors that have licenses
     using the “category” structure must select from the endorsement structure.
 
Record Change – Personnel or Responsible Managing Individual (RMI)
     Use this form to make changes to corporate officers*, LLC members* or the RMI.
 
Record Change – Independent Contractor Status
     Use this form to make changes on your CCB license record on your exempt (no employees)
     or non-exempt (have employees) status*.
 
Record Change- LBPR-RRP Certificate Holder
     Use this form to request a change to the RRP certificate holder for your Lead-Based Paint Renovation      (LBPR) Contractor's License.
 
*Changes in these areas require the licensee to complete paperwork required by other agencies. 
  (i.e. Changing your assumed business name requires you to also make the change with the
  Corporation Division.)

Residential Contractor Notices and Forms
 
Required Notices for Residential Construction (Chart)
       This chart summarizes the three notices a contractor must give to the homeowner when
       contracting for residential construction work. (Consumer Protection Notice, Information
       Notice to Owner about Construction Liens and Notice of Procedure) Copies of each of the
       forms are available to download below.
 
Consumer Protection Notice 
       This notice is required to be given to the property owner by the contractor at the time of 
       contract.
 
Information Notice to Owner About Construction Liens
       This notice is required to be given to the property owner by the contractor at the time of 
       contract.
 
Notice of Procedure 
      This notice is required to be given to the property owner by the contractor at the time
      of a written or verbal contract. It replaces the form "Owner's Duty to Contractor in the  
      Event of a Construction Dispute"
 
Homebuyer Protection Act (HPA) Notice of Compliance Form
     This document explains how the HPA, adopted by the Oregon Legislature in 2003, protects  
     buyers against construction liens filed in county records after the sale of the property where
     such liens arise out of new construction, additions or remodeling within 90 days of the date of
     the sale.
 
Notice of Right to a Lien
      This notice must be delivered to  property owners by subcontractors, material and  
      equipment suppliers before the construction is complete, though failure to deliver within 7 
      days of the project's start may reduce your lien claim.  
 
EPA "The Lead-Safe Certified Guide to Renovate Right"  |  EPA "Remodelar Correctamente" (spanish) 
      Contractors are required to provide this pamphlet prior to beginning renovation activites on
      "target housing" described in Oreon's lead-based paint requirements.
 
Model Features for Accessible Homes
      Contractors constructing new homes may provide this form containing features for
      accessibility to the home owner. It is not a required form.
 
Sample Contract for Construction Work
      The purpose of this sample contract is to serve as an educational example of the contract  
       terms that are required by ORS 701.305. It does not substitute for legal advice.
 
Contract Addendum
      This addendum is provided by the CCB to assist contractors in complying with certain
      contract term mandates. Contractors may opt to utilize or include their own language into the 
      contract that complies with the law.
 
Maintenance Schedule & Moisture Intrusion Information
      Contractors must provide information on moisture intrusion and water damage, and a  
      maintenance schedule to the homebuyer when the contractor constructs a new residential
      structure.This document may be used to comply with the requirement. Contractors may
      develop and use their own as long as it contains the information required under OAR 812-
      001-0240.
